Yes. Totally. Especially because a lot of medical records gave inaccurate information and it is important to take your own notes.

This would be so useful, as I often book apps weeks in advance and forget all the things I needed to discuss with doctor.

Section for notes for Dr Appts

When I get to the Dr it is hard to remember where the notes for her are. I end up forgetting to mention and ask things. If there was a Notes for appointments section, which I could give a date for, or not, if it isn’t made yet, then it would all be in one place. -Sarah Dufton

This is a really good idea.

In fact, a text box at the end of the tracking process that was something like “Think of something to address with a medical person?” Because honestly, I think of stuff so often and plan to write it down but forget.

But a reminder each day, with a place to put it would make such a difference.

And then some way to output the contents that includes a date of each entry (into a pdf report probably? Or even just something easily copy and pastable)

That would be fantastic.
