Support modeling of a late medicare enrollment

Medicare can start later than 65, if you’re still on an employer plan. IRMAA and Medicare Part A costs should be configurable to start later.

    Not sure how to do this, but for joint-married planning, creating independent start dates for each spouse would be ideal.
